Required Documentation for Nexus Letters
Nexus Letters establish a medical-legal link between a Veteran’s current mental health condition and their military service. To support this determination, the following documents are essential but not mandatory:
- Service Treatment Records (STRs). They provide evidence of in-service events, symptoms, or diagnoses that may relate to the claimed condition.
- DD-214 or equivalent discharge documentation that confirms service history, character of discharge, and relevant deployments or occupational roles.
- VA Rating decisions and correspondence that provides context on prior adjudications, service-connected conditions, and claim history.
- Post-Service medical records. This Includes VA and private treatment records documenting the onset, progression, and status of the condition.
- Psychiatric evaluations or psychological testing (if available) offer diagnostic clarity and support clinical formulation.

Please provide the following information before your scheduled consultation. This ensures efficient review and accurate documentation aligned with VA standards.
Identification & Service Documentation
- DD-214 or proof of military service
- VA Claim number (if available)
- Most recent Rating Decision or Current Rating
Medical & Mental Health Records

- Private provider evaluations (e.g., therapist, psychiatrist, physician)
- Past C&P exam results (especially those disputed or unclear)

- One-page summary (optional) describing how the condition affects daily life, employment, social function, or relationships (Can be written or typed, informal is acceptable)
